About the program:
Provides primary medical care for children and adolescents from birth to age 24. Health education counseling, lab services, equipment loan, school physicals, and case
management care for children with special needs are some of the services provided by the center.
ELIGIBILITY: Asthma, autism, birth defects, blind and deaf, brain damage, cancer, cardiovascular disease, cerebral palsy, chemical dependency, cystic fibrosis, delinquency,
developmentally disabled/delayed, diabetes, drug addiction, eating disorders, emotionally disturbed, epilepsy, hearing impairment, hyperkinesia, kidney disease, learning
disabilities, leukemia, lung disease, mental retardation, multiple handicaps, orthopedic impaired, physical handicaps, non-specific, seizure disorders, sickle cell anemia,
speech impairment, spina bifida, terminal illness, tuberculosis, venereal disease, and visual impairments are areas where services would be provided.
SERVICES PROVIDED: The Children's Health Center provides total health care for children, from birth to age 24. The following services are offered by the center: well-infant,
child and adolescent; case managed care for children with special needs; social worker and outreach worker; hearing tests/tympanometry, inhalation therapy/asthma treatment;
school physicals, suturing, sick care; pediatric and adolescent gynecology; lab services, developmental evaluations, equipment loan program; diabetes management; health
education counseling - cholesterol, diabetes, asthma, weight control, nutrition, family planning; and drug and substance abuse. Agency will provide referrals to specialists
and other services when indicated. Services of the center are supported via special grants for infants and children to coordinate care and to assist families to connect with
appropriate support services.
