About the program:
Provides a supplemental food program administered by the Health Center to help pregnant women, women who are breast feeding, babies, and children up to age 5.
ELIGIBILITY: Program serves infants and children up to 5 years of age, pregnant women, and women who are breast feeding up to 1 year of baby's birth, post partum women.
Program is available after screening to all nutrition risk, high risk pregnancy, those with special dietary needs for specific medical problems and low birth weight babies.
Income eligibility based on amount of income and number of people in household-evaluative scale determined by agency.
REFERRAL POLICY: Medical referral is required. It is preferred that an appointment be made. Physical examination report/medical referral is required. Verification or
certification of income is required. Medical information is required as part of screening for eligibility. Appointment must be made for screening for eligibility. Proof of
income and medical information are needed before seeing the nutritionist, who determines nutritional risk. One may call to see if income eligible. Re-certification is
required at 6 weeks post-partum and 6 months post-partum if breast feeding and at 6 month intervals for infants and children.
SERVICES PROVIDED: WIC is a supplemental food program administered by the U.S. Dept. of Agriculture to help pregnant women, postpartum women who are breast feeding, and babies
and children up to age five, obtain foods needed for better health. Vouchers, which are valid only for purchase of certain foods, are issued to the client. The prescribed
foods are those which are good sources of protein, minerals, especially iron and calcium, and vitamin C. If a financial problem is discovered in the course of the admission
screening inverview, client is referred to social services for possible furhter assistance. Health and nutrition education counseling is included in the certification for the
WIC program.
