About the program:
A new choice of long term care for frail seniors, assists the frail elderly to remain as independent as possible by providing a complete program of health care and health
related services, all designed to keep the individual living at home in their community. Acts as an alternative to nursing home institutionalization, and serves frail elders
who are nursing home eligible but who prefer to remain at home. Medicaid, Medicare and private pay are accepted. All health care and health related services are managed by a
multi-disciplinary team of professionals.
