About the program:
Provides individualized, comprehensive home-based services to pregnant women and their families. The program promotes optimal child development and enhancing parent-child relationships. The family receives services through the child's fifth year. The home visitor assesses family strengths and needs and develops a case plan with the family. The home visitor listens and provides emotional support and offers suggestions and role modeling in problem solving. Parents receive instructions related to effective parenting techniques and home management skills. Home visitors link families with a pediatric medical provider to ensure that the baby receives well-baby checks and immunizations. Families recieve assistance with obtaining community services and support as they work towards self-suffiency.
