About the program:
Provides information on how to obtain birth and death certificates; simple name corrections; and declaration of paternity. ELIGIBILITY: Medicaid Services-must have current Medicaid card, doctor's appointment only for covered services. Vital records-must be related, attorney or creditor to qualify for death certificate. SERVICES PROVIDED: Norfolk Health Department Vital Statistics Division has birth and death records as follows-birth records from 1900 thru June 1912. Birth records after June, 1912 are with The Bureau of Vital Statistics, Virginia Health Department, 109 Governor Street, Richmond, Virginia 23219. Norfolk Health Department Vital Statistics Division Office will not mail forms for applying for copies of birth certificates, but will give them to clients who come to pick them up. Birth certificates can be requested without use of the form. Death records from 1900 on are maintained at Norfolk Health Department Vital Statistics Office. This office also provides Medicaid transportation (bus tickets) for qualified Medicaid recipients who call four days in advance of their appointments.
