About the program:
Provides individualized educational and residential services and in-community supports for children, youth, and adults with severre emotional disturbance, learning
disabilities, mental retardation, autistic disorder, behavioral disorders, and other complex challenges, including physical disabilities.
ADDITIONAL INFORMATION: Offerings include and accredited high school, elementary and middle school graded classrooms, ungraded classrooms, vocational training and participation
in public school. Grafton operates the largest private residential educational program in Virginia on numerous campuses and provides program development/technical assistance
at sites all over Virginia, the District of Columbia, Maryland, and West Virginia. Grafton offers consultation services, diagnostic services, and a therapeutic foster care
program with short term, long term, and respite care services.
ELIGIBILITY: Aggressive/assaultive behavior; autism disorder; behavior/conduct disorder; delinquent/court involvement; developmental disability; emotional disturbance;
family-related needs; fire starter; health/medical impairment; incarcerated parent(s); learning disability; mental retardation; orthopaedic impairment; poor academic
performance; physical/emotional abuse/neglect; runaway; substance abuse; sexually abused; sensory impairment; speech/language impairment; sex offender; suicidal indication or a
history of suicide.
ELIGIBILITY RESTRICTIONS: Individuals with major medical problems that require continuous medical oversight or treatment; individuals who are currently suicidal or homicidal;
individuals who require treatment for drug or alcohol abuse; individuals who are active fire-setter, Grafton may determine inappropriate for admission; indviduals in an acute
psychotic condition; individuals whose behavior endangers themselves or others.
REFERRAL POLICY: Referral is by self, guardians, professional or agecny. Students may be referred from any source, and a complete packet of admissions materials will be sent
to any potential referral source upon request. A pre-placement interview/conference is required with sponsors, guardian, and individuals following receipt of appropriate
referral information. Appropriateness of a student interviewed is decided by the preplacement team. The following information is required prior to involvement and will be
maintained in the students master file after enrollment:
1. Grafton application forms
2. Grafton preliminary history forms
3. Psychological report which is not more than two years old
and includes:
a) current behavioral functioning/social competence;
b) current emotional status
4. Social history report which includes:
a) family history/relationships;
b) social/developmental history
5. Educational/Vocational reports which include a current IEP
and transcripts.
6. Physical examination form including a Hepatitis B-Antigen
Screening.
7. Dental examination form.
8. Physician-signed immunization record.
9. Hearing evaluation.
10. Vision evaluation.
11. Medical history which includes:
A) past or present significant medical problems
B) previous treatment for mental health, mental
retardation, substance abuse and behavior problems.
12. Drug usage profile which includes:
A) the use of psychotropic or anti-convulsant medications
B) the use of prescription and non-prescription drugs
being taken at the time of admission & for the previous
6 months.
C) drug allergies, idiosyncratic and other adverse drug
reactions.
D) ineffective chemotherapy
13. Necessary release forms.

diagnostic services; educational services; home-based services; independent living services; residential services; rehabilitative services; respite care services;
