About the program:
Provides an integrated delivery system of quality services to meet the needs of the frail elderly and their families to prevent premature institutionalization, physical and emotional decline and promote an optimal quality of life. Services include counseling and case management, homecare, companion, and skilled nursing services, personal affairs management, volunteer services, meals on wheels, assessment and monitoring services for elders with family out of town.
