About the program:
Provides services to assist persons age 60 and over to remain as independent as possible for as long as possible. This comprehensive program acquires, coordinates and monitors services older adults need to remain in their homes. SERVICES PROVIDED: Information and referral, comprehensive assessment, care plan development/goal setting, coordination of community resources and information support systems, monitoring of services, and reassessment of client needs as necessary.
