About the program:
Provides an in home comprehensive assessment of the elder individual or couple, care plan, linkage to services, monitoring, and advocacy. The program assesses the elder's medical, functional, personal, and social needs. The main intent of the program is to enable the elder individual to remain safely in the home with the support necessary to maintain qualily of life.
