About the program:
Provides health care coordination and primary care for children from birth to age 5. Services include provision of each child with a medical home by a pediatrician or family practitioner, case management by an experienced public health nurse to ensure referral to other health and social agencies, and outreach services for assistance with housing, education, etc. thereby improving health care and promoting self sufficiency.
